Sakamoto Ryouta è un disoccupato di 22 anni che vive con sua madre. Nel mondo reale, potrebbe non esserci nulla di veramente speciale in lui, ma online è uno dei migliori giocatori al mondo del gioco di combattimento Btooom !. Un giorno, si sveglia su quella che sembra essere un'isola tropicale, anche se non ha memoria di come o perché sia arrivato lì. Mentre vaga, Ryouta vede qualcuno e chiede aiuto. Lo sconosciuto risponde lanciandogli una bomba! Ora Ryouta si rende conto che la sua vita è in pericolo e che in qualche modo è rimasto intrappolato in una versione reale del suo gioco preferito! Ryouta riuscirà a sopravvivere abbastanza a lungo da capire come e perché è finito qui?
